Switching Between Thyroid Medications: Generic to Brand, Tablet to Liquid, and Beyond

Any switch between thyroid medications — generic to brand, brand to brand, or tablet to liquid — needs a TSH recheck at 6 weeks. Levothyroxine has a narrow therapeutic index, so small absorption differences shift hormone levels meaningfully.

Why switching medications matters more for thyroid than for most drugs

A 2010 pharmacovigilance survey from a joint ATA/AACE/TES task force analyzed 199 adverse events reported on levothyroxine therapy and found that about 89% occurred after a switch between products, with approximately 92% of those substitutions made by pharmacies without notifying the prescriber [C2]. What recovers when you stay on a consistent product
