THE SHORT ANSWER

How long to wait after levothyroxine

Take levothyroxine on an empty stomach with water. Wait at least 60 minutes before coffee or food, and about 4 hours before calcium, iron, soy, fiber supplements, or antacids.

SubstanceMinimum waitWhy
Coffee60 minutesCoffee can reduce absorption if taken too soon after the dose.
Any food or breakfast60 minutesFood in the stomach slows and lowers how much medication is absorbed.
Calcium & dairy4 hoursCalcium binds levothyroxine into a form the gut can't absorb.
Iron supplements4 hoursIron forms an insoluble complex with levothyroxine.
Soy4 hoursSoy protein interferes with absorption in the gut.
Fiber supplements4 hoursFiber traps the medication and carries it through undigested.
Antacids4 hoursAluminum, magnesium, and calcium salts bind the dose.
Walnuts4 hoursTheir fiber and phytate content can blunt absorption.

WHY THESE WINDOWS

Grounded in how the drug is absorbed

Levothyroxine is absorbed in the small intestine, and that absorption is easily disrupted. Coffee and food shorten the window and lower the dose your body actually receives, while calcium, iron, and similar minerals bind the medication directly. Spacing them out protects a steady, predictable dose.

COMMON QUESTIONS

Levothyroxine timing FAQ

Can I take levothyroxine at night?

Yes. Many people take it at bedtime, at least a few hours after their last meal. The same spacing idea applies: keep it away from food and mineral supplements. Pick one consistent time and stick with it, and confirm the approach with your clinician.

Does decaf coffee count?

Treat decaf the same as regular coffee and wait about 60 minutes. The interaction is linked to compounds in the coffee itself, not only the caffeine.

What about water?

Plain water is fine and even recommended. Take your tablet with a full glass of water — water does not interfere with absorption.

Why 4 hours for calcium and iron?

Calcium and iron bind levothyroxine in the gut and form a complex your body can't absorb, which can meaningfully lower your dose. A roughly 4-hour gap lets the medication absorb first.

Do I need to wait before taking my other daily pills?

It depends on the pill. Minerals like calcium and iron and some antacids need the 4-hour gap, while many other medications do not. Ask your pharmacist about your specific list.
